Application for Emigration to Europe

Instructions for English application: Save a copy of the downloaded application and elder's recommendation forms in a folder on your hard disk. Open the saved application in Adobe Acrobat Reader or the PDF viewer of your choice and fill out the form by typing text directly into the form and clicking in the appropriate check boxes. Save the filled-out application as a PDF and send the application by email to LME. If you are unable to save it as a PDF, print the filled-out application and mail, fax, or submit it in person to the LME office in Anaheim, using the mailing address and fax phone number given in the application. If you fax the application, send an email to LME to notify us of this (in case the fax is not received).

Be sure to print a copy for your records. If you are unable to fill out the application by typing text directly into the form, print the blank application and fill it out by hand. After making a copy of the application for yourself, mail or fax it, using the mailing address and fax phone number given in the application. You may also submit it in person to the LME office in Anaheim or, if you are able to scan the application (preferably to PDF) or otherwise create a PDF of the filled-out application, send it to LME via email.

Important: The recommendation by the elders or responsible brothers should be filled out, signed, and sent by the brothers to the LME office separate from the application. An application will be considered complete only after this recommendation has been received by LME.
